Preventive Health Screening for Low Income Children and Families
Children First, A Merage Foundation is recruiting active and retired volunteer pediatricians for an innovative health assessment program at El Sol Academy, a unique Charter School in Santa Ana (www.elsolacademy.org). The goal is to expand health care services to the children and families of El Sol by enrolling them in the newly established El Sol Children Learning Center (FCLA). The health screening is not a comprehensive physical examination but a screening that may result in additional visits to the health screening practitioner, a referral to the client's medical home, a referral for social services or a referral to a medical specialist.
Program Summary:
- Each family will have approximately 45 - 60 minutes for the health assessment
- Physicians will identify health problems with the aim of disease prevention, health improvement and quality of life
- Link patients and family to a medical home
- Refer to CAAs for insurance enrollment and to Promotora (case managers) for follow-up and referrals to community agencies
- Assess children's immunization history and make appropriate referrals for immunizations
- Help educate families about family health including nutrition, exercise and regular examinations
- Screening tools available for all physicians
- Nursing staff is eligible to participate
- Flexible dates and times for health screening (week days, evenings & weekends available)
